The Chronic Care Ecosystem — Navigating the Path to Wellness

Collaboration
Deep-Dive Immersion & Stakeholder Interviews
Rather than designing in a silo, I collaborated directly with telehealth teams (doctors and health coaches) to map existing workflows, identifying friction points where technology could scale human coaching versus where the human touch was mandatory.
Firsthand Empathy Building
I enrolled in the program myself as a "user" across various coaches and conditions, experiencing the onboarding friction firsthand to pinpoint structural irregularities and see how health plans were adapted in real time.
Cross-Functional Whiteboarding
Facilitated intensive FigJam whiteboarding sessions with co-founders, product managers, and developers to align on technical constraints, define standard operating timelines (3-month vs. 6-month plans), and structure backend data syncing to eliminate repetitive document requests.

Design Exploration
From Paper to Systems
Moved from initial paper wireframes and competitor mood boards to structured user flows in FigJam for affinity mapping.
Design Systems & Cross-Platform Scalability
Built a unified component library in Figma to ensure design consistency across both the Web App (Coach Dashboard) and Mobile App (Patient Facing).
Standardized data-heavy tracking modules, metric cards, and input fields using advanced auto-layout and component properties.
Created clear documentation and handoff guidelines for developers, ensuring smooth cross-platform implementation between the web dashboard and mobile interface.

Design Solution
The final ecosystem was structured into a cohesive multi-platform architecture:
The Health Coach Dashboard (Web App)
A centralized command center synced with user apps, allowing coaches to manage multiple patients, review automated medical histories, and eliminate manual note-taking errors.
Intuitive Short Onboarding & Categorization
Partnered with health coaches to audit questionnaire items—categorizing them into essential, optional, and subjective buckets—drastically reducing initial onboarding friction and eliminating tedious 20-minute setup calls.
Smart Health Reports & AI-Driven Engagement
Designed comprehensive health report layouts to make diagnostic data digestible for users, alongside temporary AI-generated health plans to maintain user engagement between coach check-ins.
Standardized Operational Workflows
Established reusable modular frameworks supporting flexible program timelines (3-month and 6-month tracks) tailored to specific chronic conditions.

Outcomes and Learnings
Impact & Quantified Success
Structured & Traceable Data: Replaced fragmented, manual coach notes with a centralized data repository, eliminating data loss during staff transitions and ensuring seamless multi-patient tracking.
Simplified Progress Tracking: Standardized standard operating procedures (SOPs) and modular health frameworks made it significantly easier for users to visualize and track their day-to-day lifestyle improvements.
Reduced Cognitive Load: Streamlined onboarding questionnaires replaced exhausting 20-minute verbal interrogations with quick, categorized inputs, dramatically improving user sentiment from day one.
Retrospective & What I Would Do Next
Next Steps: While the ecosystem successfully aligned coaches and patients, a future iteration should focus on deeper automated telemetry integration (wearable syncing) to reduce manual check-in burdens even further.
